How to Become a My Choice Wisconsin Healthcare Provider

As of January 1, 2020, Care Wisconsin and My Choice Family Care merged into one entity now known as My Choice Wisconsin.

As we continue to work through the process of integrating operations and evaluating our combined network adequacy, we have temporarily paused long-term care contracting, including adding new locations, rate reviews, and new AFH certifications. Our anticipated timeframe to re-open the long-term care network for contracting is January 2022.

We will update this page with any changes to this timeline. Thank you for your patience as we work through this exciting time!

Current Providers

We know that many residential and supportive visit providers are anxious to discuss the status of their contract. We are diligently working to complete re-contracting with approximately 5000 providers, and as of April 20th, we have completed approximately 2500 recontracts.

If your organization has not yet been contacted, please know:

• Your current contract will remain in force until a new agreement is in place.
• We expect to make contact with all affected service providers over the next 3 months (by the end of May).
• We ask that you do not call your contracting specialist or customer service center to check when you will be contacted. This information cannot be provided.
• We will contact you as soon as we possibly can; time spent on status calls diminishes our capacity to complete new contracts in a timely manner.

We will provide updated status on re-contracting progress the week of June 14th, so please check back on this website for new information.

In the summer of 2019, My Choice acquired Trilogy Health Insurance and on June 1, 2021 rebranded it under My Choice Wisconsin Health Plan, Inc.

What does this mean for you? Beside the name change to My Choice Wisconsin Health Plan, Inc. – nothing. There are no changes to contracts, to the process for authorization requests, or your contacts.
